DFA REQUIREMENTS FOR RENEWAL OF PASSPORT

For those with expired or expiring passport who wish to travel abroad soon, you need to renew your passport in the Department of Foreign Affairs or DFA.

As your international travel date approaches, make sure to check the validity of your passport. Immigration law requires that your Philippine passport must be valid at least 6 months on your travel date.

Demo 1: If your fly date to Hong Kong is December 2018, your passport will expire by March 2019 (3 months from December 2018), immigration officer will not allow you to board your plane.

Demo 2: Your trip to Hong Kong will be on December 2018, your passport will expire by August 2019 (8 months from December 2018) you will be allowed to fly.

Here is the list of requirements for Philippine passport renewal:

GENERAL REQUIREMENTS

  • Personal appearance of passport applicant.
  • Confirmed appointment for passport processing. (There are certain cases allowed for walk-ins, please read further below for details.)
  • Duly accomplished passport application form which may be downloaded from the DFA website. See link at the bottom.
  • Most recent expiring or expired passport.
  • Acceptable IDs at least one (1): for Brown, Green and MRP (without IC Chip logo). Please refer to List of Acceptable IDs.
  • Other Supporting Document as necessary

LIST OF ACCEPTABLE IDS (Present at least 1 of the following if you are renewing passport, present at least 2 if you are a first-timer.):




Government-issued picture IDs such as the following:

  • Digitized SSS ID
  • Driver’s License
  • GSIS E-card
  • PRC ID
  • IBP ID
  • OWWA ID
  • Digitized BIR ID
  • Senior Citizen’s ID
  • Unified Multi-Purpose ID
  • Voter’s ID
  • Improved Postal ID (PHLPost  Improved Postal ID is now accepted as Primary ID in passport application. The (old) basic Postal ID which is the laminated paper version can’t be used as Primary ID. UPDATE 2018: Per checking on the official website of DFA, Digitized Postal ID is not considered as Primary but if you go to official website of Phlpost the digitized postal ID is still considered as Primary ID)

*NOTE – If it’s your first time to apply for a PH passport, recommended ID to bring is government-issued digitized IDs. Bring as many as you have just to be sure so you won’t waste time, money, and effort in going to the Department of Foreign Affairs (example: Voter’s ID, UMID or SSS ID, Driver’s License. etc.)

Other acceptable picture IDs such as the following

  • Old College ID
  • Alumni ID
  • Old Employment IDs

LIST OF SUPPORTING DOCUMENTS (Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant, at least 2 of the following if you are a first-timer; if renewing no need to present supporting docs if you can present a valid government ID as listed above together with your expired/expiring passport):

    • PSA Marriage Contract
    • Land Title
    • Seaman’s Book
    • Elementary or High School Form 137 or Transcript of Records with readable dry seal
    • Government Service Record
    • NBI Clearance
    • Police Clearance
    • Barangay Clearance
    • Readable SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
    • Voter’s Certification, List of Voters and Voter’s Registration Record (please attach receipt)
    • School Yearbook
    • Paper based Postal ID

 *ADDITIONAL REQUIREMENTS*

For holders of Brown Passport or passports issued prior to 01 May 1995:

      • Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page.
      • Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA, to determine complete middle name.
      • If PSA Birth Certificate is blurred or unreadable; transcribed certified true copy of the birth certificate from the Local Civil Registry is required when entries in PSA Birth Certificate are blurred or unreadable.
      • If born abroad, Report of Birth duly authenticated by PSA

For holders of Green Passport or passports issued after 01 May 1995:

PH Green Passport

PH Green Passport

      • Bring original green passport and photocopy of the data page and last page of passport.
      • Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA, to determine complete middle name
      • If PSA Birth Certificate is blurred or unreadable; transcribed certified true copy of the birth certificate from the Local Civil Registry is required when entries in PSA Birth Certificate are blurred or unreadable.
      • If born abroad, Report of Birth duly authenticated by PSA

 

For holders of Machine Readable Passport (MRP without IC Chip logo) issued in the year 2007:

      • Bring original passport and photocopy of data page and last page of passport
      • Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA, to determine complete middle name
      • If PSA Birth Certificate is blurred or unreadable; transcribed certified true copy of the birth certificate from the Local Civil Registry is required when entries in PSA Birth Certificate are blurred or unreadable.

For holders of Electronic Passport (E-passport)

Bring original e-passport and photocopy of page 2 (data page).

 

 

For women who would change their civil status from single to married:

      • Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.
      • If entries are blurred or unreadable, submit Transcribed Marriage Contract from the LCR
      • If married abroad, Report of Marriage duly authenticated by PSA
      • Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (This is only required for women with foreign national husband married in the Philippines).

For these special cases, kindly inquire direct from DFA for the passport renewal requirements:

      1. For married women who would like to revert to their maiden name.
      2. For minor applicants, requirements for first time minor applicants are still prescribed, in addition to the passport of minor.
      3. For Applicants who availed of Dual Citizenship under RA 9225.
      4. For Applicants who were naturalized.
      5. For Applicants who elected Philippine Citizenships.
      6. For Muslim Applicants
      7. For divorced women

A. For Regular Individuals, only the following group are ALLOWED for walk-ins:

There is a special priority lane “Courtesy Lane Facility” for passport applicants who are:

      • Seniors
      • PWDs
      • Pregnant Women with medical certificate
      • and Minors (7 years old and below) accompanied by a Parent/Guardian

For this group allowed for walk-in, you can download the DFA passport application form. Make sure that you completely fill-up the form before submitting it to the DFA office together with all the other requirements.

For the rest of us, we need to schedule an appointment for passport renewal and new passport application. We need to learn how to schedule an appointment in DFA for passport.

      • Good evening po. May passport ako issued 2015. My ex fiance get me a visa and naapprove siya pero d KO sya nagamit dahil ng.away kami at pinunit nya yung page 20 yung may visa stamp.

        Ngayon I try to get new passport kasi d ba damage na yung passport KO. Nung ng.inquire ako sa DFA Dumaguete, sabi nila pagawa daw ako ng affidavit of mutilation but nung ng.apply na ako, sabi naman na dapat kunin KO raw yung napunit na page. D KO nman alam if buhay paba yung ex fiance KO.. And the option daw is punta daw ako manila embassy.. Ganun ba yun?.. Wala nabang ibang option? Please I need some information what I need to do.

        • Hi Realyn, you really need to issue a Notarized Affidavit of Mutilation, in the affidavit indicate therein that you can no longer find the missing pages since they were thrown away. Check this post on the full requirements for mutilated/damaged passport.

          If your concern is about the visa granted to you and you want your passport to be stamped with the visa again, you really need to go to the Manila Embassy per advise of the DFA personnel. But if your concern is only to get a replacement passport, then just follow the requirements and procedure of DFA to get a replacement for mutilated passport.

  5. Just a little correction: under Demo 1, March 2016 should be March 2017 & under Demo 2, August 2016 should be August 2017.

  7. Please kindly help, clarify, & advice on the best option for me here’s my scenario.
    1. I’ve lost my brown passport sometime 2011 in there was still my maiden name(maria phyllis imelda caballero dimalanta.) ive secured an “affidavit of loss but not a police report. i still have not replaced that passport.
    2. I have gotten married since then, my last name is now (Price) & also is now a widow.(Mr. Price died)
    3. So if i replace this passport before year end of 2016, do i revert back to my unmarried last name or?please advice & what are the documents i need to secure. further, am now a disabled person & all your advice & assistance on this matter is greatly appreciated.

    • Hi Maria, since yours is a green passport and has expired, you do not need to file a police report, a notarized affidavit of loss will suffice in ADDITION to all other requirements for green-passport-holder-renewal.

      About the name that you will use – this will depend on what you are currently using legally in your everyday transaction. Our recommendation would be to call DFA office po so you can ask them all the questions directly and get the answers right away.

  8. Hi. I will be renewing my passport. My old.one is a machine readable passport issued in 2009. One of the additional reqts is the birth cert in security paper issued by the Philippine Statistics Authority. What i jave now is issued by the NSO. Is this still acceptable or i have to get a new one issued by the PSA? Thank you.

    • Hi Lou, we consulted your query with a DFA personnel and here’s the reply: We still accept NSO copies as long as its original, readable and in good condition.

Hi good afternoon po for renewal na po ang passport ko Kaya Lang Hindi ko mare new Dahil wala akong latest na I’d yong last na renew ko 2010 ang Sss I’D na ginamit ko single pa ako Pero Kinuhanan ako ng marriage copy from pso Dahil mag change na ako ng status . Ngaun gusto ko ulit I renew ang passport ko wala akong Philippine I’D na married na ako Kundi yong old employment id ex ofw po ksi ako.pwede ko po ba Kaya I present yong Sss id ko na single ang status ang nakalagay.

    • Hi Revelyn, yung passport na hawak mo now is E-Passport na, right? Yung may chip sa harap? If it is, you just need to bring that, iphotocopy mo yung page 2. Then also submit yung Marriage Contract in Security paper (SECPA) or certified true copy of Marriage Contract issued by the local registrar and duly authenticated by the PSA. Tapos, bring mo all your old IDs bearing your single name, esp yung government issued like SSS, dalhin mo din yung married-name-old-company ID mo. Balitaan mo kami pagkatapos mo mag-renew. Salamat!

  10. i would like to ask about the passport of my brother, his passport has been expired last oct 2015 and he would like to renew it but unfortunately he lost his passport.. what we do is we file an affidavit of loss.. and i schedule him an appointment to renew his passport this oct 28 2016… Are they accepting the affidavit of loss since that his passport is expired and loss??? what else are the requirements in renewing passport…??

    Gusto ko lang pong malaman kung pwede ko pang i-renew ang aking passport issued last Dec.3,2000 at nag expired po ng Dec.3,2005 .ang problema ko po ang birth certificate ko iba sa ginagamit kong family name now ko lang na-check sa municipality of calamba city kung saan ako pinanganak…nakakuha po ako ng passport last 1995 pa at sa abroad ko na ito napa-renew till 2004 ng makauwi ako ..till then di ko na ito napa-renew …please advice me what to do and many thank.

    • Hi Roland, pasensya na talaga but on your case, you really need to go direct sa DFA and talk to a DFA officer. Medyo komplikado kasi yang case mo, malamang hingan ka nila ng additional documentations.

    • Hi Lyra, yes you need an authorization coming from the mother or father of the child, on top of requirements for first-time passport applicants.

Hi, just wanna say, we’ve been there at dfa cebu, we want to “walk-in” for having a renewal. we want to avail the 10-15 days of processing ’cause my partner must have his medical for his job, but sad to say, they didn’t allow us to have walk-in. they told us to have appointment which causes the medical delayed. as I read this page, it’s stated that it is okay to do walk-in in our case. then why the guard insisted us to have appointment first? what should we do?

    • Hi Debby, if your partner has a medical emergency, he can walk-in. Bring a medical certification that says it’s urgent for him to travel abroad to seek medical help. Also, if you go any government agency, or any establishment for that matter, talk to an officer, not the guard. The guard’s duty is on security.

    • You can renew your Passport at the DFA Ortigas office… OFW’s need not have online appointment, walk in lang pwede na if you are to go back sa same work abroad.

Hello, I did not completed yet my appointment schedule and suddenly there’s an internet interruption says this site cannot be reach, i started again but the website replies my appointment is already exist and instructed to cancel my previous appointment, but i didn’t recieve any email from DFA for confirmation so i can use the appoinment code which require upon cancelling appointment.     Just want to inquire the following cases:
    1. My passport issued last 2012 and expired this january 2017. Am i going to renew my passport or considered as a new applicant?

    2. I will also apply for a passport of my two year old girl. Her birth certificate issued by the PSA is enough requirement for the application? Kasi pg binabasa ko po.mga requirements for minors, kelangan ng valid passports of both parents. Mgrrenew pa po ako ng passport at ang ang aswa ko.po ay ngttrabaho.abroad.

  34. Hello ask ko lang kong ano pwede gawin para maka kuha ang anak ko ng passport na gamit na ang apilido ng ama. Since nag school sya apilido na ng ama gamit nya. Kaht sa BC nya ay apilido ko pa. na legitimize nya last November 2016. kaso hinde pa sya na issue ng NSO kasi kailangan pa ma correct ang mga pangalan namin sa marriage contract na mali mali na aabot pa sa 4 months na proseso at hinde lang po yun kulang ang pangalan ko sa BC nya na kailangan din ilagay at i file 4 months ulit ang hihintayin.. ok na sa civil registrar ng city hall ang legitimation.     • Hi, one of the requirements for a passport is NSO Birth Certificate…. kung ano po ang complete name nung bata na nakalagay dun, yung po ang pangalan na gagamitin sa passport.

  41. Good day!we have a question about renewal of passport. My mom have her passport expired in 1999. She needs to renew it. Here’s the issue..she didnt know that her given name on her birth certificate came with Maria. She only found this out when she first applied for passport. She had a notarized affidavit before but she cant find it anymore. She doesnt have a single ID with Maria on it.
