REQUIREMENTS to Replace Lost / Damaged / Mutilated Passports

Is your passport lost or mutilated? Learn how to file for replacement and the requirements that needs to be submitted to the Department of Foreign Affairs or DFA.

GENERAL REQUIREMENTS FOR LOST PASSPORTS:

These are the general requirements in getting a replacement for lost passports.

  • Personal appearance.
  • Confirmed appointment for applicants with expired passports and issued travel documents.

Lost-valid passport applicant may proceed directly to the Consular Records Division at DFA-Aseana without an appointment or as a “walk-in”.

ADDITIONAL REQUIREMENTS:

In case your passport was lost and cannot be found:

  • You need to submit a Notarized Affidavit of Loss (with detailed explanation on when, where and how passport got lost).
  • If the passport you lost is still valid, you also need to submit Police Report.

*If the passport you lost is expired, there is no need to submit police report.

  • Photocopy of first page of lost passport (if you have a copy available)

Applicants have to take note that there will be a 15-days clearing period prior to the processing of application for replacement of a lost valid passport.

Now, what if you have already declared to the DFA that your passport was lost but you found it later or it was returned to you within the 15-day-clearing period, what should you do?

  • You need to submit a notarized Affidavit of Retrieval (with detailed explanation on when, where and how passport was found)
  • Original and photocopy of data page and last page of passport.

MUTILATED OR DAMAGE PASSPORTS

In case your passport was damaged or mutilated and you want to file for a replacement of it, these are the requirements you need to submit the DFA:

  • Personal appearance
  • Confirmed appointment
  • Submit Notarized Affidavit of Mutilation (with detailed explanation on when, where and how passport got mutilated or damaged)
  • Original and photocopy of first and last page of mutilated or damaged passport

APPLICANTS WITH ISSUED TRAVEL DOCUMENT

In case you were issued a travel document, and you now want to file for a replacement for your cancelled passport, these are what you need to submit:

  • Personal appearance
  • Confirmed appointment
  • Original Travel Document
  • Submit Notarized Affidavit of Explanation Regarding Issuance of Travel Document (with detailed explanation on when, where and why applicant was issued a travel document)
  • Original and photocopy of data page and last page of cancelled passport (if passport is still in the possession of the applicant)
Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.

Returning OFWs and seafarers may walk-in for passport processing. The DFA Robinson’s Galleria is the passport center dedicated to OFWs.

    Pag mutilated yung passport dahil nabasa (naisama sa labada), kahit saang branch po ba pwedeng iparenew? Wala na kasing available schedule for the month of january sa DFA Aseana. eh magkocontract signing na ako next week for a work abroad. Gusto ko sana sya iprocess na this week kaso walang mga available schedules. Thanks!

Hi just want to ask,nandito ako ngayon sa Australia at yung passport ko nabasa din at lumabo konti yung ibang details and kailangan kong umuwi soon, naka book ako sa katapusan, pumunta ako ng Philippine consulate para mg pa issue ng travel document, at sabi nila doon hindi daw ng aallow ng travel document ang cebu pacific kaya ngtanong ako mismo sa Airport ng sydney (service desk) kung pwede b yung travel document ko sabi sa akin pwede naman daw basta ipakita ko ung letter,kasi may pinayagan sila that day ng mkauwi with the same situation (may travel document) as me, I’m still worried since sabi ng consul PAL at quantas lang daw ang pwede, my passport was not cancelled kc hndi naman sobrang damaged, need some advice please.

  9. Wala po bang ibang paraan para makakuha ng passport for lost complaint, kasi in 10 days may tour kami sa hongkong provided ng company na pinapasukan ko. meron po ba?

    • try niyo po patulong sa travel agency but in our opinion ang 10 days allowance ay di po kakayanin. talagang paschedule kayo via online and will submit all required documents for Lost passports. Tapos you have to wait more for the passport to be released ng dfa.

    • Lost passport na po yang sa inyo, parang new passport iapply plus yung police report or notarized affidavit of lost. Pakibasa po sa article sa taas kasi naka link dun ang info about requirements need mo iprepare and ang procedure.

  14. Hello I have a very important to ask. It’s about my sister who exited Canada because the government didn’t grant her the refugee claim. She came legally here but what happen is that the first employer release her because she didn’t need her service anymore then we found another employer but right before she can get the permit the old lady that she is supposed to take care died and the her daughter release her again because her mom died already until it gets more complicated. Now she applied for refugee at the end but didn’t grant her so they let her go out from the country. They took her passport just a copy of it with some corresponding documents. Now she is in the Philippines how can she get or renew her passport again can u please tell me in details what she has to do.

    • Hi Estrella, since your sister’s case has become complicated, we strongly recommend that you go visit a DFA branch to consult with an officer.

  15. Good day, ask ko lang po pano po kaya kung upon delivery na-damaged yung passport? nagpa-deliver po ksi ako sa LBC since meron naman po sa loob ng dfa sm manila. Pero pagdting po sa akn nung passport yung mismong plastic ng LBC may butas na at pag kita ko sa passport ko damaged na yung pinaka cover. Pano po kaya mainam k0ng gawin.salamat

Hi po ask ko po lost passport po ako nakapag file na po ako.isa po akong ofw pabalik na po ako ng August 12 kaylangan ko po makabalik kasi po mafinish na po ung visa ko ng August 17 kaya need ko po nakabalik ng same date ng fight ko pag d po ako makabalik mawawalan po ako ng trabaho ng file po ako sa dfa manila ng lost passport pinabalik po ako ng August 3 bumalik po ako dala ko ung mga requirements ko kaso kulang po ako ng id un lang po ung kulang ko pro d nila accept kaylangan ko raw ng id.eh wala po akong id dalang id naiwan ko po don sa bharain.diba priority nyo ang ofw at d ko naman po ginusto na mawala ung passport ko sana mabigyan nyo ko ng tulong salamat po
