Is your passport lost or mutilated? Learn how to file for replacement and the requirements that needs to be submitted to the Department of Foreign Affairs or DFA.

GENERAL REQUIREMENTS FOR LOST PASSPORTS:
These are the general requirements in getting a replacement for lost passports.
- Personal appearance.
- Confirmed appointment for applicants with expired passports and issued travel documents.
Lost-valid passport applicant may proceed directly to the Consular Records Division at DFA-Aseana without an appointment or as a “walk-in”.
- Duly accomplished passport application form.
- Requirements for the first-time passport applicants are still prescribed.
ADDITIONAL REQUIREMENTS:
In case your passport was lost and cannot be found:
- You need to submit a Notarized Affidavit of Loss (with detailed explanation on when, where and how passport got lost).
- If the passport you lost is still valid, you also need to submit Police Report.
*If the passport you lost is expired, there is no need to submit police report.
- Photocopy of first page of lost passport (if you have a copy available)
Applicants have to take note that there will be a 15-days clearing period prior to the processing of application for replacement of a lost valid passport.
Now, what if you have already declared to the DFA that your passport was lost but you found it later or it was returned to you within the 15-day-clearing period, what should you do?
- You need to submit a notarized Affidavit of Retrieval (with detailed explanation on when, where and how passport was found)
- Original and photocopy of data page and last page of passport.
MUTILATED OR DAMAGE PASSPORTS
In case your passport was damaged or mutilated and you want to file for a replacement of it, these are the requirements you need to submit the DFA:
- Personal appearance
- Confirmed appointment
- Submit Notarized Affidavit of Mutilation (with detailed explanation on when, where and how passport got mutilated or damaged)
- Original and photocopy of first and last page of mutilated or damaged passport
APPLICANTS WITH ISSUED TRAVEL DOCUMENT
In case you were issued a travel document, and you now want to file for a replacement for your cancelled passport, these are what you need to submit:
- Personal appearance
- Confirmed appointment
- Original Travel Document
- Submit Notarized Affidavit of Explanation Regarding Issuance of Travel Document (with detailed explanation on when, where and why applicant was issued a travel document)
- Original and photocopy of data page and last page of cancelled passport (if passport is still in the possession of the applicant)
