It was found out that there are thousands of e-Passports handed out by the DFA (Department of Foreign Affairs). These passports have serial numbers from EB0000001 to EB1267350.
These passports are found out to be defective for their covers are easily peeled of or pages are falling off.
If you have one of these passports, you can have them replaced by visiting the Office of Consular Affairs (DFA-Aseana) in Macapagal Avenue, Parañaque City, the nearest Satellite Office (if residing in Metro Manila), or the nearest Regional Consular Office. The DFA will check whether your passport is substandard or not.
Philippine e-Passport
The DFA will replace your e-passport for free if:
- If the applicant will fill up the required passport replacement forms
- If upon investigation and examination the e-Passport is deemed substandard
- If the holder surrenders the detached e-Passport to the Department
Defective passport holders can be charged for replacement if:
- The majority of the passport’s pages have been used already
- The e-Passport’s condition is the result of wear and tear or impreper handling and storage
- If the holder of the detached e-Passport refuses to surrender said passport to the Department of Foreign Affairs

nabasa po kasi yun nung bagyo sa ami sa leyte..humuhingi po ako ng sorry kung bakit diko po agad na dala sa DFa yung pasport ko pag ka dating ko po dito sa manila..nung nag apply po ako ng job papuntang abroad.naka pasa po ako sa employers interview then they said na kailangan kung patingnan yung pasport ko..then one of the staff of DFA aseana said that i need to get an apointment and get also a afidavit of mutilation..so now i have it.ant tommorow is my apoinment…please assist niyo naman po ao tommorow para po di masayang ticket ko…at para po sa pamilya ko tong gagawin o..i want to help them para po maka bangon…maraming salamat po..brian athony dolamos
